Kevin Fernandez, DPT
Dr. Fernandez received his Doctorate of Physical Therapy Degree in 2019 at the University of St. Augustine. Before receiving his DPT, Dr. Fernandez attended Florida International University and earned a Bachelor of Science degree in Sports and Fitness Studies. His passion for orthopedics and sports performance is what drives him to get his patients to achieve their goals. Prior to becoming a clinician, he had the privilege of working with all age groups. Born and raised in Miami, Dr. Fernandez loves to play sports and be by the water. As a clinician, Dr. Fernandez practices evidence-based research and is patient centered. His treatment approach relies heavily on manual therapy techniques, therapeutic exercises, movement performance and patient education.

Kevin Valledor, DPT
Dr. Kevin Valledor earned his Doctorate in Physical Therapy from the University of St. Augustine for Health Sciences in 2024, following a Bachelor of Science in Athletic Training from Florida State University. As a lifelong athlete with a deep-rooted passion for basketball, Dr. Valledor brings a unique perspective to the rehabilitation process while blending clinical knowledge with firsthand understanding of athletic performance and recovery.
Kevin is known for his compassionate approach and commitment to personalized care. He utilizes a combination of manual therapy and movement-based interventions to help individuals restore strength, improve mobility, and regain confidence in their bodies. Whether you’re healing from an injury or striving to reach your peak performance, Kevin is dedicated to guiding you every step of the way with empathy, expertise, and encouragement.

Javier Pérez Mena, PTA, CMPTA
Javier received his Associate’s Degree as a Physical Therapist Assistant in 2014 from the The Praxis Institute in Miami, Florida. He also completed a certification in manual therapy sanctioned by Florida Institute of Orthopedic Manual Physical Therapy (FIOMPT) in 2017. FIOMPT is a consortium of Physical therapists that have advanced training in specialized areas with peer reviewed clinical evidence-based research. Javier is motivated in perusing new learning opportunities throughout his career to achieve the best quality patient care. He chose this profession after realizing he could help others by providing them with the right education to achieve their optimal level of function. Javier’s passion is influenced by his love for sports and fitness.

Janely Perez, PTA
Janely specializes in manual therapy and functional exercise, tailoring each treatment plan to meet the unique needs of her patients in order to support recovery and long-term wellness. Her calm, supportive approach and deep clinical knowledge have made her a trusted guide for those on the path to recovery.
Committed to lifelong learning and professional growth, Janely is currently pursuing her Bachelor of Science degree to further elevate her practice and continue delivering the highest standard of care. Her mission is to create a welcoming and encouraging environment where every patient feels seen, supported, and empowered to reach their goals.

Henry Diaz-Morales, PTA
Henry’s passion for physical therapy began with a personal experience while he was recovering from a sports-related back injury. It was through that process that he witnessed the profound impact physical therapy can have not only on the body, but on a person’s confidence, independence, and overall well-being.
Today, Henry brings that same empathy and understanding to every session. His goal is to help each patient feel supported, motivated, and empowered as they work toward healing and their own recovery journey. Whether you’re recovering from an injury or looking to regain strength and mobility; Henry is committed to helping his patients return to the activities they love through providing compassion, encouragement, and guidance every step of the way.

Nelson Perez, PTA
Nelson Perez is a licensed and registered Physical Therapist Assistant with a strong foundation in patient-centered care. A graduate of Florida National University, he also holds a Bachelor of Science in Natural Science and brings over four years of hands-on experience in physical therapy. Nelson has worked with a wide range of patients, helping individuals recover from orthopedic injuries, neurological conditions, age-related challenges, and sports-related issues.
Known for his compassionate approach and attentive care, Nelson is committed to supporting each patient’s unique healing journey. His background also includes seven years of experience as a Phlebotomy Technician in hospitals and clinics, further enriching his understanding of patient care across different healthcare settings.
Currently pursuing his Doctor of Physical Therapy degree, Nelson remains deeply dedicated to professional growth and excellence. His goal is to help every patient feel seen, supported, and empowered while not only restoring movement, but also elevating their confidence and quality of life.

Jasmin Areces, MSOT
Jasmin is a registered and licensed Occupational Therapist who received her Master’s in Occupational Therapy at the University of St. Augustine for Health Sciences, where she graduated with honors. Before beginning her masters, Jasmin graduated with honors from Florida International University with a bachelor’s degree in Recreational Sport Management. Jasmin decided to pursue a career in orthopedic rehabilitation while volunteering at local therapy centers. She strives to help others regain independence in their daily activities while using various therapeutic exercises and activities to further improve their quality of life. In her spare time Jasmin enjoys traveling, watching movies, and spending time with her closest friends and family. As a therapist, Jasmin’s goals are to provide each client with an individualized treatment plan based on the client’s own personal goals while using evidence-based practice.

Aaron Dos Santos, PTA
Aaron earned his Associate of Science degree as a Physical Therapist Assistant from Keiser University in 2020. With a strong foundation in orthopedic care, he specializes in post-operative rehabilitation and return-to-sport therapy, helping patients of all ages regain strength, confidence, and function.
Drawing from a lifelong involvement in team sports and coaching, Aaron brings energy, creativity, and relatability to every session. He has a special talent for connecting with his patients and guiding them toward their goals in a way that feels both encouraging and achievable. His experience as a youth volleyball coach enhances his ability to motivate and empower, not only those working to get back to competitive or recreational activities, but also individuals looking to obtain a better quality of life.
Aaron is also known for creating a welcoming and positive environment in the clinic. His light-hearted approach is not just effective, but helps patients feel at ease and makes the recovery
process more enjoyable. Above all, Aaron is passionate about helping others return to the activities that bring them joy, and he’s committed to supporting each individual with compassion, professionalism, and a touch of humor along the way.

Alberto Blanco, MSOT
Alberto Blanco received his first Master’s of Science from FIU in Sports Management with a concentration in Therapeutic Recreation and his second Master’s in Occupational Therapy from the University of St. Augustine. He was a Director of Recreational Therapy for a 100-bed hospital facility where he tailored and created fitness and wellness programs for patients of all ages. His journey into occupational therapy was influenced by his ability to make meaningful connections and improve overall quality of life. Alberto, also has a broad knowledge on improving sports-related and orthopedic conditions through functional strength training and therapeutic interventions, while developing and achieving high level performance outcomes. He is currently finishing his Doctorates in Occupational Therapy and strives to impact and enhance the lives of his patients through evidenced based practice.
Gyasi Azpeitia, OTR/L and Eduardo Rodriguez
Founders of Liv Active Therapy Center in 2016.
Gyasi graduated from Florida International University in 2003 with a Bachelor of Science Degree in Occupational Therapy. She has over 20 years of experience working in and administrating outpatient rehabilitation centers. Her passion for her profession and for helping people has made her one of the top Occupational Therapists in Miami.
Eduardo has run numerous businesses throughout his career. He has a background in strength training and conditioning. Having trained numerous athletes throughout his career, he has seen and assisted in the recovery of many sports related injuries. Having himself suffered injuries as a young athlete, he understood the importance of having the finest rehabilitative care provided to ensure the best outcome and return to sport.
Together they opened Liv Active Therapy to provide the highest quality of care to all people in a positive and compassionate environment with services provided by highly skilled and knowledgeable therapists. They understand the importance of individualized care and maintain their centers to the highest standards.
