Kevin Fernandez PT, DPT

Dr. Fernandez received his Doctorate of Physical Therapy Degree in 2019 at the University of St. Augustine. Before receiving his DPT, Dr. Fernandez attended Florida International University and earned a Bachelor of Science degree in Sports and Fitness Studies. His passion for orthopedics and sports performance is what drives him to get his patients to achieve their goals. Prior to becoming a clinician, he had the privilege of working with all age groups. Born and raised in Miami, Dr. Fernandez loves to play sports and be by the water. As a clinician, Dr. Fernandez practices evidence-based research and is patient centered. His treatment approach relies heavily on manual therapy techniques, therapeutic exercises, movement performance and patient education.

Javier Pérez Mena, PTA, CMPTA

Javier received his Associate’s Degree as a Physical Therapist Assistant in 2014 from the The Praxis Institute in Miami, Florida. He also completed a certification in manual therapy sanctioned by Florida Institute of Orthopedic Manual Physical Therapy (FIOMPT) in 2017. FIOMPT is a consortium of Physical therapists that have advanced training in specialized areas with peer reviewed clinical evidence-based research. Javier is motivated in perusing new learning opportunities throughout his career to achieve the best quality patient care. He chose this profession after realizing he could help others by providing them with the right education to achieve their optimal level of function. Javier’s passion is influenced by his love for sports and fitness.

Ener Garcia, PTA

Ener Garcia, PTA

Ener holds an Associate of Science degree in Physical Therapist Assistant from Keiser University, awarded in October 2020. He brings a specialized focus in orthopedic and musculoskeletal injury rehabilitation, post-surgical recovery, and sports function restoration, skills honed through collaborative work with esteemed doctors of physical therapy, orthopedic specialists, and sports medicine physicians. Before his tenure at Liv Active, Ener amassed substantial experience as a strength and conditioning coach, where he tailored one-on-one training programs to optimize individual physical fitness, and facilitated sports performance enhancement in both youth and adult athletes. 

Ener is a staunch advocate for patient-centered care, consistently prioritizing the best interests of those he serves. He is a proponent of engaging patients as active participants in their recovery journey, believing that empowerment through education and collaboration is key to optimal health outcomes. With a philosophy that integrates enhancing general fitness and wellbeing into his therapeutic approach, Ener is dedicated to not only aiding in rehabilitation but also to elevating the overall quality of life for his patients. His treatment method is characterized by a holistic strategy that tailors to the individual’s goals, fostering an environment where improved function and enhanced wellbeing are the shared objectives.

His credentials are further enhanced by certifications as a Certified Personal Trainer, a Corrective Exercise Specialist, and a Performance Enhancement Specialist, which are all accredited by the National Academy of Sports Medicine. Ener prides himself on integrating his comprehensive background in performance training with evidence-based practices to excel as a proficient clinician and a dedicated rehabilitation specialist.

Claudia Morales, PTA

Claudia Morales PTA completed her Associate in Science degree as a Physical Therapy Assistant from Miami Dade College in 2021. Prior to completing her degree, Claudia was an athlete who played volleyball and badminton. During her sport career she suffered injuries which led her to obtain rehabilitation services. Through her experiences she grew a passion for the field of Physical Therapy. Her clinical experience include and are not limited to; pediatrics, orthopedics, and neurological conditions. More specifically, Claudia has a high clinical interest in movement science to prevent musculoskeletal injuries and promote tissue healing. Also, with the intent of being an advocate for patients to facilitate recovery; Claudia also completed her Massage Therapy degree from Miami Dade COllege in 2022.

Luz A. Gonzalez, COTA

Luz A. Gonzalez licensed Certified Occupational Therapist Assistant who graduated from Keiser University in 2018. Prior to attaining her degree, Luz knew the setting she was passionate about and pursued opportunities in an outpatient rehabilitation setting for 6 years to gain the skills and knowledge to ensure her success. Luz has dedicated her time to specializing in orthopedic and neurological conditions of the upper extremity. Luz is an enthusiastic, hardworking, dedicated team player committed to delivering the highest quality of care.
Logan Spiroff, PTA

Logan Spiroff, PTA

Logan is a licensed and registered Physical Therapy Assistant, whose career began with graduation from Miami Dade College in 2018. He had amassed his comprehensive experience and eduction amongst elite therapists and orthopedic professionals throughout the course of his career and has honed his skills in rehabilitative treatments and patient recovery to individuals of all ages. 

Logan has also acquired many advanced techniques during time in which he worked with children with neurological deficits, further expanding his skill set and understanding of specialized care. 

What sets Logan apart is his exceptional communication skills, which allow him to connect with patients on a personal level, fostering trust and understanding throughout their therapeutic journey. His empathetic approach and genuine concern for his patients’ well-being reflect his commitment to improving lives through individualized care and treatments. 

NELSON PEREZ, PTA

Nelson Perez, PTA

Nelson Perez is a licensed and registered Physical Therapist Assistant graduated from Florida National University. Leading with passion and a broad knowledge in his field, Nelson has made a positive impact in the lives of many patients throughout his life not only as a PTA but also as a Phlebotomist Technician having worked at many hospitals and different clinics over a 7-year period. He is currently working on a Bachelor of Science degree in Natural Science at FNU to pursue a Doctorate degree in Physical Therapy. Over the years, Nelson has gained valuable experience working closely with patients recovering from orthopedic, neurological, geriatric and sport related conditions.

Jasmin Areces, MSOT

Jasmin is a registered and licensed Occupational Therapist who received her Master’s in Occupational Therapy at the University of St. Augustine for Health Sciences, where she graduated with honors. Before beginning her masters, Jasmin graduated with honors from Florida International University with a bachelor’s degree in Recreational Sport Management. Jasmin decided to pursue a career in orthopedic rehabilitation while volunteering at local therapy centers. She strives to help others regain independence in their daily activities while using various therapeutic exercises and activities to further improve their quality of life. In her spare time Jasmin enjoys traveling, watching movies, and spending time with her closest friends and family. As a therapist, Jasmin’s goals are to provide each client with an individualized treatment plan based on the client’s own personal goals while using evidence-based practice.

Danielle Llopiz, MSOT

Danielle is a licensed and registered Occupational Therapist. She completed her Masters In Occupational Therapy degree from Florida International University in 2020. Danielle also has a Bachelor’s degree in Exercise Science with a concentration in coaching, from Ithaca College in Ithaca, NY. Danielle has worked in health and wellness as a personal trainer, group fitness class instructor, as well as nutrition coach for over 6 years. Danielle has experience training individuals in functional fitness as well as Olympic Weightlifting and is Level 2 Trainer Certified in Crossfit. Danielle has a passion for movement and strong interest in treating upper extremities. Through occupational therapy treatments, Danielle strives to allow her patients to live a functional and independent life, regardless of disability or injury they may be suffering from. When Danielle is not working, she enjoys spending time working out, playing with her dog, and traveling with her partner.

Daniel Machado, PTA

Daniel received his associate degree as a Physical Therapist Assistant in 2020 from The Praxis Institute in Miami, Florida. Daniel enjoys fitness, mountain biking, and traveling. Prior to joining Liv Active Therapy, Daniel was a group fitness trainer for 7 years where he led CrossFit and Bootcamp classes. Through his experience as a trainer, he developed a passion to promote health and wellness to all age groups and promote a better way of life. Now, as a physical therapist assistant, he thrives to improve patients’ quality of life through evidence-based practice, functional movements, and manual therapy techniques. He continuously educates himself in his field and attends specialty courses to provide better all-around care to his patients.
Alex Lage, DPT, OCS, MTC, CSCS

Alex Lage, DPT, OCS, MTC, CSCS

Dr. Alex Lage boasts over two decades of experience in Orthopaedics and Sports Medicine Physical Therapy. Over the years, he has been a trusted practitioner for a plethora of athletes across various professional leagues including the NFL, NBA, MLS, MLB, NHL, PGA and ATP. 

Dr. Lage’s expertise extends to treating athletes from renowned neighboring universities such as the University of Miami, Florida International University, Barry University, and Nova University, along with recreational athletes and individuals from diverse backgrounds. His paramount objective is to empower his clients to optimize their quality of life while circumventing the need for surgeries and painful injections and other interventions. 

Alex commenced his academic journey by graduating Summa Cum Laude with a Bachelor’s in Biology and exercise physiology from Barry University in 1995. He furthered his education by obtaining a Master’s in Physical Therapy, graduating at the top of his class in 1997. His pursuit of excellence led him to attain an advanced Master’s in Manual and Orthopedic Physical Therapy in 1999 from the Institute of Graduate Physical Therapy. Dr. Lage’s dedication to his field culminated in a Doctorate in Physical Therapy from the University of St. Augustine in 2003.

In 2001, Dr. Lage achieved board certification in Orthopedics from the American Board of Physical Therapy Specialties. He has been recognized as a certified Strength and Conditioning Specialist by the National Strength and Conditioning Association since 1998. Adding to his repertoire of skills, Dr. Lage obtained certification in Dry Needling through the Florida Institute of Orthopedic Manual Therapy in 2020.

Presently, Dr. Lage is on the verge of completing an Osteopratic diploma certification with the American Academy of Manipulative Therapy. This rigorous program encompasses Diplomas in Spinal Manipulative Therapy (2023), Dry Needling (2024), and Vestibular Rehabilitation (2024).

Outside his professional endeavors, Alex indulges in recreational bodybuilding, securing multiple local and state titles. He also serves as a nutrition coach, aiding numerous clients in reclaiming their fitness and leading pain-free lives.

Dr. Lage’s commitment to service extends beyond his profession, as he proudly served in the United States Marine Corps, embodying the values of discipline and dedication.

Alberto Blanco, MSOT

Alberto Blanco received his first Master’s of Science from FIU in Sports Management with a concentration in Therapeutic Recreation and his second Master’s in Occupational Therapy from the University of St. Augustine. He was a Director of Recreational Therapy for a 100-bed hospital facility where he tailored and created fitness and wellness programs for patients of all ages. His journey into occupational therapy was influenced by his ability to make meaningful connections and improve overall quality of life.  Alberto, also has a broad knowledge on improving sports-related and orthopedic conditions through functional strength training and therapeutic interventions, while developing and achieving high level performance outcomes.  He is currently finishing his Doctorates in Occupational Therapy and strives to impact and enhance the lives of his patients through evidenced based practice. 

Gyasi Azpeitia, OTR/L and Eduardo Rodriguez

Gyasi graduated from Florida International University in 2003 with a Bachelor of Science Degree in Occupational Therapy. She has over 20 years of experience working in and administrating outpatient rehabilitation centers. Her passion for her profession and for helping people has made her one of the top Occupational Therapists in Miami.

Eduardo has run numerous businesses throughout his career. He has a background in strength training and conditioning. Having trained numerous athletes throughout his career, he has seen and assisted in the recovery of many sports related injuries. Having himself suffered injuries as a young athlete, he understood the importance of having the finest rehabilitative care provided to ensure the best outcome and return to sport.

Together they opened Liv Active Therapy to provide the highest quality of care to all people in a positive and compassionate environment with services provided by highly skilled and knowledgeable therapists. They understand the importance of individualized care and maintain their centers to the highest standards.
